What to Test, Why It Matters, and How to Use the Results Like a Grown Man
Once you hit 50, bloodwork stops being “just a check-up.”
It becomes your dashboard.
You may feel fine, train hard, run a business, lead a family, and still have silent problems building under the hood: insulin resistance, rising cardiovascular risk, low testosterone, fatty liver, chronic inflammation, nutrient deficiencies, kidney strain, or early prostate issues.
The goal is not to become obsessed with numbers.

The goal is simple: catch problems early, fix what you can control, and build a stronger second half of life.
Below is the 50TOUGH Bloodwork Checklist for men over 50.
First Rule: Don’t Just Get “Basic Labs”
Most standard physicals include a few useful tests, but they often miss the markers that matter most for men over 50.
A basic annual panel may say you’re “normal,” while your insulin is climbing, ApoB is too high, inflammation is elevated, testosterone is crashing, or your liver is showing early signs of fatty change.
“Normal” does not always mean “optimal.”
It usually means you are inside a broad reference range based on the general population — and the general population is not exactly the picture of peak male health.
The Core 50TOUGH Bloodwork Panel
1. Complete Blood Count — CBC
What it tells you
A CBC checks your red blood cells, white blood cells, haemoglobin, haematocrit, and platelets.
This gives insight into:
Anaemia
Infection
Immune function
Inflammation
Blood thickness
Recovery capacity
Possible bleeding issues
Potential blood disorders
Key markers to review
Haemoglobin
Haematocrit
Red blood cell count
White blood cell count
Platelets
MCV, MCH, MCHC
Why men over 50 need it
Low haemoglobin may point toward anaemia, nutrient deficiency, chronic disease, or blood loss. High haematocrit can happen with dehydration, sleep apnoea, smoking, testosterone therapy, or other conditions.
If you are on testosterone replacement therapy, haematocrit is especially important because TRT can increase red blood cell production in some men.
2. Comprehensive Metabolic Panel — CMP
What it tells you
The CMP is one of the most useful foundational blood panels. It checks kidney function, liver function, electrolytes, blood protein levels, and glucose.

Creatinine
eGFR

Sodium
Potassium
Chloride
Calcium
Albumin
Total protein
AST
ALT
Alkaline phosphatase
Bilirubin
Why it matters
This is your “engine room” panel.
It tells you how your liver, kidneys, hydration status, blood sugar, and electrolyte balance are doing.
For men over 50, this panel can pick up early signs of:
Fatty liver
Kidney stress
Poor hydration
Blood sugar problems
Medication side effects
Alcohol-related strain
Metabolic dysfunction
Watch especially
ALT and AST are liver enzymes. They may rise from alcohol, fatty liver, medications, intense training, or liver injury.
Creatinine and eGFR help assess kidney function. If eGFR is consistently below 60 for at least three months, that may indicate chronic kidney disease and needs medical follow-up.
3. Fasting Glucose, HbA1c, and Fasting Insulin
This is one of the most important sections for men over 50.
Fasting glucose
This measures blood sugar at one point in time.
General categories:
Normal: under 100 mg/dL
Prediabetes: 100–125 mg/dL
Diabetes range: 126 mg/dL or higher on repeat testing
HbA1c
HbA1c estimates average blood sugar over roughly three months.
General categories:
Normal: below 5.7%
Prediabetes: 5.7–6.4%
Diabetes range: 6.5% or higher on repeat testing
Fasting insulin
This is not always included in standard labs, but it can be extremely useful.
Why?
Because insulin often rises years before glucose or HbA1c become abnormal.
That means a man can have “normal blood sugar” while his body is working overtime to keep it there.
Why it matters
Insulin resistance is tied to:
Belly fat
Fatty liver
Low energy
Brain fog
Higher cardiovascular risk
Erectile dysfunction
Higher inflammation
Poor testosterone status
Increased diabetes risk
If your waist is growing and your energy is fading, do not just look at glucose. Look deeper.
4. Lipid Panel
Standard markers
Total cholesterol
LDL cholesterol
HDL cholesterol
Triglycerides
Non-HDL cholesterol
Why it matters
Heart disease is still one of the biggest threats to men over 50. You need to know your cardiovascular risk before symptoms show up.
General targets to understand
Triglycerides: ideally below 150 mg/dL; many metabolically healthy men are under 100
HDL: above 40 mg/dL in men is generally considered acceptable, but higher is not always automatically protective
LDL: target depends on your total risk profile
Non-HDL cholesterol: often a better risk marker than LDL alone, especially when triglycerides are elevated
The 50TOUGH view
A basic lipid panel is useful, but it is not enough.
You also want ApoB.
5. ApoB — Apolipoprotein B
What it tells you
ApoB measures the number of atherogenic particles in your bloodstream — the particles that can enter artery walls and contribute to plaque build-up.
These include:
LDL particles
VLDL particles
IDL particles
Lp(a) particles
Why ApoB matters
LDL cholesterol tells you how much cholesterol is carried inside LDL particles.
ApoB tells you how many dangerous particles are actually present.
That matters because two men can have the same LDL cholesterol number but very different particle counts — and therefore different risk.
General interpretation
Targets depend on your risk level, but many preventive cardiology groups use rough goals such as:
Lower-risk men: ApoB below 90 mg/dL
Higher-risk men: ApoB below 80 mg/dL
Very high-risk men: ApoB below 65 mg/dL
Your physician should help determine your target based on your age, blood pressure, diabetes status, smoking history, family history, coronary calcium score, and other risk factors.
6. Lipoprotein(a) — Lp(a)
What it tells you
Lp(a) is a genetically influenced cholesterol particle that can significantly increase cardiovascular risk.
Why men over 50 should check it
You usually only need to test Lp(a) once or twice in your life because it is mostly genetic.
If it is high, lifestyle alone usually does not lower it much — but knowing it changes how aggressive you may need to be with other risk factors.
Common high-risk cut-off
Greater than 50 mg/dL, or
Greater than 125 nmol/L
Different labs use different units, so do not compare mg/dL directly with nmol/L.
Why it matters
If your father had a heart attack at 52, your cholesterol looks “not bad,” and nobody has checked your Lp(a), you are missing a big piece of the puzzle.
7. hs-CRP — High-Sensitivity C-Reactive Protein
What it tells you
hs-CRP is a marker of systemic inflammation.
General interpretation
Low risk: below 1.0 mg/L
Average risk: 1.0–3.0 mg/L
Higher risk: above 3.0 mg/L
If hs-CRP is above 10 mg/L, it may reflect an infection, injury, or acute inflammatory issue. It should usually be repeated when you are well.
Why it matters
Chronic inflammation is associated with cardiovascular disease, obesity, insulin resistance, autoimmune disease, poor recovery, gum disease, sleep apnoea, and overtraining.
This is not a standalone diagnosis, but it is a useful smoke alarm.
8. Thyroid Panel
Minimum test

Important note
TSH is usually the first-line screening test. A typical reference range is roughly 0.4–4.5 mIU/L, though interpretation depends on symptoms, age, medications, and the lab.
Do not self-diagnose thyroid disease from one number. Thyroid treatment should be managed by a qualified clinician.
9. Testosterone Panel
Men over 45 should not guess about testosterone.
They should measure it correctly.
Minimum testosterone testing
Total testosterone
Free testosterone or calculated free testosterone
SHBG
Albumin
If testosterone is low, add

Testosterone should generally be checked:
In the morning
Ideally before 10 a.m.
When you are not acutely sick
After decent sleep if possible
Repeated on a separate day if low
Most guidelines define low testosterone as consistently low levels, often total testosterone below about 300 ng/dL, combined with relevant symptoms.

Low libido
Erectile dysfunction
Loss of morning erections
Low drive
Depression or irritability
Loss of muscle
Increased belly fat
Poor recovery
Low bone density
Fatigue
The 50TOUGH warning
Do not jump on testosterone because you feel tired.
You may have sleep apnoea, insulin resistance, high stress, too much alcohol, poor training recovery, depression, thyroid issues, or nutrient deficiencies.
Testosterone therapy can be life-changing for the right man. It can also be the wrong tool if the real problem is elsewhere.
10. PSA — Prostate-Specific Antigen
What it tells you
PSA is a blood marker used in prostate cancer screening and prostate health monitoring.
Who should discuss PSA screening
Guidelines vary slightly, but the modern approach is shared decision-making.
Many men should discuss PSA screening with their doctor/physician around age 45–50.
Men at higher risk may need that conversation earlier, including those with:
Black ancestry
Strong family history of prostate cancer
Known high-risk genetic mutations
A father or brother diagnosed with prostate cancer, especially at a younger age
Important
PSA can rise from prostate cancer, but also from benign prostate enlargement, inflammation, infection, ejaculation, cycling, or recent prostate manipulation.
A high PSA does not automatically mean cancer. It means follow-up is needed.
11. Vitamin D
What it tells you
The test is called:
25-hydroxy vitamin D
General interpretation
Deficiency: commonly below 20 ng/mL
Possible insufficiency: often 20–30 ng/mL
Many clinicians aim for roughly 30–50 ng/mL, depending on the person
Why it matters
Vitamin D is involved in bone health, immune function, muscle function, and overall metabolic health.
Who should especially check it
Men with low sun exposure
Men with darker skin living in northern climates
Men with osteoporosis or fractures
Men with obesity
Men with malabsorption issues
Men taking certain medications
Men with chronic kidney or liver disease
Routine screening in every asymptomatic adult is debated, but for many men over 50 it is a useful optimisation marker.
12. Vitamin B12 and Folate
Why it matters
Low B12 can affect:
Energy
Nerve function
Mood
Memory
Balance
Red blood cell production
Higher-risk men
Men on metformin
Men taking acid blockers long-term
Vegans or strict vegetarians
Men with gut issues
Men with heavy alcohol intake
Men with unexplained anaemia or neuropathy
Common interpretation
B12 below 200 pg/mL is often considered deficient
200–300 pg/mL may be borderline and may need methylmalonic acid testing
If symptoms are present, do not ignore a borderline result.
13. Iron Studies and Ferritin
Key tests
Ferritin
Serum iron
TIBC
Transferrin saturation
Why it matters
Iron can be too low or too high.
Low iron may suggest poor intake, absorption issues, or blood loss.
High ferritin can reflect excess iron, inflammation, liver disease, metabolic syndrome, or hemochromatosis.
Men over 50 warning
Iron deficiency in a grown man deserves attention. It can sometimes point toward gastrointestinal blood loss and should be evaluated properly.
Do not just take iron because you feel tired.
Test first.
14. Uric Acid
What it tells you
Uric acid is associated with gout, kidney stones, kidney function, and metabolic health.
General high range
Above about 7.0 mg/dL in men is often considered elevated
Why it matters
High uric acid often travels with:
Belly fat
High blood pressure
Insulin resistance
Alcohol intake
High fructose intake
Kidney strain
Gout risk
Even if you do not have gout, elevated uric acid can be a useful metabolic warning sign.
15. GGT — Gamma-Glutamyl Transferase
What it tells you
GGT is a liver enzyme that can rise with:
Alcohol use
Fatty liver
Bile duct issues
Oxidative stress
Certain medications
Why it matters
AST and ALT can look normal while GGT gives an earlier clue that your liver is under pressure.
If you drink regularly, carry belly fat, or have high triglycerides, GGT is worth checking.
16. Kidney Protection Add-On: Urine Albumin-to-Creatinine Ratio
This is not bloodwork, but it belongs in the same conversation.
What it tells you
The urine albumin-to-creatinine ratio, or UACR, checks for small amounts of protein leaking into the urine.
Why it matters
It can detect early kidney damage before standard kidney blood markers look abnormal.
This is especially important if you have:
High blood pressure
Diabetes or prediabetes
Obesity
Cardiovascular disease
Family history of kidney disease
General interpretation
Normal: below 30 mg/g
Moderately increased: 30–300 mg/g
Severely increased: above 300 mg/g
The 50TOUGH Advanced Performance Panel
These are not mandatory for every man, but they can be useful depending on symptoms, goals, and risk profile.
Homocysteine
High homocysteine may relate to B vitamin status, cardiovascular risk, kidney function, genetics, and inflammation.
Common goal ranges vary, but many clinicians prefer to see it under about 10–12 µmol/L.
Omega-3 Index
This measures EPA and DHA levels in red blood cells.
Higher omega-3 index levels are associated with better cardiovascular profiles. Many experts consider 8% or higher a desirable range, though this test is not universally required.
Magnesium
Serum magnesium is commonly tested but does not always reflect total body magnesium well. RBC magnesium may provide more insight, though availability varies.
Magnesium matters for blood pressure, glucose control, sleep, muscle function, and heart rhythm.
Cortisol
Cortisol testing can be useful in specific situations, but it should not be randomly interpreted. Stress is real, but cortisol biology is complex.
If you suspect adrenal or endocrine problems, work with a clinician.
DHEA-S
DHEA-S may be checked as part of a broader hormone evaluation, especially when fatigue, low mood, or endocrine dysfunction is suspected.
Again, context matters.
How Often Should Men Over 50 Get Bloodwork?
At minimum
Most men over 50 should consider comprehensive bloodwork once per year.
Every 3–6 months if you are actively correcting a problem
This includes:
Prediabetes
Diabetes
High ApoB or cholesterol
Fatty liver
Low testosterone treatment
Kidney issues
High blood pressure
Major weight-loss phase
New medication
Major lifestyle intervention
One-time or occasional tests
Lp(a): usually once, unless your clinician repeats it
Genetic risk markers: as needed
Advanced cardiovascular testing: based on risk
How to Prepare for Bloodwork
If you want clean data, do not sabotage the test.
The day before
Avoid alcohol for 24–48 hours
Avoid brutal training for 24–48 hours
Hydrate normally
Do not eat a huge late-night meal
Sleep as well as possible
Morning of testing
Fast 8–12 hours if checking fasting glucose, insulin, and triglycerides
Water is usually fine
Take medications only as directed by your doctor/physician
Test testosterone in the morning
Avoid ejaculation and cycling before PSA testing if your doctor advises, since both can temporarily affect PSA
Do not do this
Do not stop prescribed medication without medical guidance
Do not test right after illness and assume the numbers reflect your baseline
Do not compare your results to a random influencer’s “optimal range”
Do not panic over one abnormal marker
Do not ignore repeated abnormal trends
The Big Five Men Over 50 Cannot Ignore
If you only remember five things, remember these.

These are not “old man tests.” These are leadership tests. You are taking ownership of the machine you live in.
Red Flags That Need Medical Follow-Up
Do not “biohack” these away.
Talk to your physician if you see:
HbA1c in diabetes range
Fasting glucose repeatedly 126 mg/dL or higher
eGFR below 60
Significant protein in urine
Markedly elevated liver enzymes
PSA above expected range or rapidly rising
Very high triglycerides, especially above 500 mg/dL
Unexplained anaemia
Very high haematocrit
Very high ferritin or transferrin saturation
Low testosterone with high prolactin
hs-CRP persistently high without explanation
Chest pain, shortness of breath, fainting, neurological symptoms, or severe fatigue
Lifestyle Moves That Improve the Most Blood Markers
The same boring basics work because biology rewards consistency.
1. Build muscle
Lift 3–4 days per week.
Muscle improves glucose control, insulin sensitivity, testosterone environment, bone density, posture, and long-term independence.
2. Walk daily
Aim for 7,000–10,000 steps per day, adjusted to your fitness level.
Walking is underrated medicine for glucose, blood pressure, mood, and fat loss.
3. Eat protein like an adult
Most men under-eat protein and overeat processed carbs and alcohol.
A practical target for many active men is roughly 0.7–1.0 grams of protein per pound of ideal body weight per day, adjusted for kidney health and medical conditions.
4. Control alcohol
Alcohol can worsen:
Sleep
Testosterone
Liver enzymes
Triglycerides
Blood pressure
Belly fat
Recovery
Mood
You do not need to be perfect. But you do need to be honest.
5. Sleep like it matters
Because it does.
Poor sleep can crush testosterone, increase hunger, worsen insulin resistance, raise blood pressure, and damage recovery.
If you snore, wake up gasping, or feel exhausted despite enough hours in bed, get evaluated for sleep apnoea.
6. Fix your waistline
For many men, waist circumference is one of the clearest signs of metabolic health.
If your waist is climbing, your labs usually follow.
7. Stop smoking and nicotine dependence
Smoking dramatically increases cardiovascular, cancer, lung, and vascular risk. If you smoke, quitting is one of the highest-return health decisions you can make.
